The ALW Covers:
- Bathing, grooming, dressing, and other daily personal care
- Medication managemen
- Meals and housekeeping
- Secure memory care for people with dementia or Alzheimer's
- Wellness monitoring and care management program support
You may be eligible for ALW if you:
- Are at least 21 years old.
- Have Medi-Cal with no share of cost.
- Need nursing home-level care.
- Prefer to live in assisted living facilities in California.
- Help moving from a hospital or facility into assisted living
- Housing and placement support
- Ongoing case management and coordination
- Behavioral health and wellness services
- Licensed ALW provider and CalAIM partner
- 24/7 care in a beautifully secure and welcoming environment
- Memory care specializes in Alzheimer's care and dementia
