Nursing Facility Transition

Lafayette Gardens Assisted Living Facility takes great pride in being part of the California Advancing and Innovating Medi-Cal (CalAIM) Nursing Transition Program. Our goal is to provide seamless transitions for individuals moving from skilled nursing facilities (SNFs) (60 days stay) to our supportive,home-like environment. With this program, residents receive comprehensive care services across medical, social, and personal needs.

• Comprehensive Transition Support: We team up with SNFs, case managers, and Medi-Cal managed care plans to make sure the process is smooth for residents.
• Individualized Care Plans: Every resident is thoroughly assessed to allow the development of a care plan specific to that resident, addressing medical, mental health, and social needs.
• Medical and Behavioral Health Services: Access to primary care, mental health professionals, and other healthcare services within our provider network. 
• Personalized Assistance: Our caregivers are trained to assist with the activities of daily living (ADLs), medication management, and management of chronic illnesses.
• Enhanced Care Coordination: Our staff collaborates with interdisciplinary teams to ensure continuity of care and to prevent any possible readmissions to the hospital.
• 24/7 Supervision & Support: Lafayette Gardens provides 24/7 assistance to guarantee the safety and well-being of all residents.

Residents transitioning under the CalAIM program must meet the following requirements:

• Be enrolled with Medi-Cal and CCHP.
• Require assistance with ADLs or with complex medical needs.
• Being transitioned from a skilled nursing facility, hospital, or some other institutional environment.
• Requires long-term supportive housing in an assisted living environment.
